If AssistedCare Services, LLC offers you and you accept a position, you will
be required to:

Be at least 18 years of age.
Have a High school diploma or equivalent (GED).
Have relevant experience and/or personal knowledge of disabilities (preferred).
Three personal references for PCA position (two written references required for Chore and Respite
Be familiar with various types of disabilities (preferred).
Be committed to the belief that persons with disabilities have the ability to direct their own care and live
independently in their own homes with the proper assistance.
Have reliable/safe transportation and current valid driver's license.(copy of license will be made)
Provide proof of automobile insurance with the following minimum liability: $300,000 Combined Single
Limit Liability Bodily Injury & Property Damage OR $100,000 Bodily Injury Per Person/$300,000 Bodily
Injury Per Accident/$50,000 Property Damage Per Accident/Split Limit Liability.(copies will be made)
Provide Current Division of Motor Vehicle (DMV) driving record.
Submit a check payable to the State of Alaska for a criminal background check.  The cost for the
background check is $79.25. ($54.25 for state processing and  FBI processing and $25.00 for online
processing). Two sets of fingerprints (one set for local and another for federal check) are also needed.

Provide valid current CPR and First Aid certifications.
Provide proof of negative TB (Tuberculosis) test or chest x-ray (preferred but not required).
Provide Social Security Card. (copies of documents will be made by AssistedCare Services, LLC and
originals will be returned to you).    
Complete Form I-9 (Employment Eligibility Verification) / and W-4 tax form.
Complete all conditions of employment prior to your first day of work including mandatory training  
      and if applicable, provider enrollment in the new SDS MMIS system
**Optional: You can opt to have all fees deducted from your first or multiple paychecks    
after you start working with a client.
